After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. PC & Google Chrome Users
Our online form uses the latest version of Adobe Acrobat Reader to conveniently submit the form from home or work. Please download the free plug-in from Adobe's web site if it is not already installed on your system. It is important that you have at least version 9 of the plug-in to successfully use our online form. Google Chrome users will need to change their settings to open the form using Adobe Reader:
You will need to disable the Chrome PDF viewer and revert to Adobe Reader, follow these steps:1. Type "about:plugins" in the address bar (without the quotes)
2. Hit "enter" key (on your keyboard)
3. Click on "details" (near the top right)
4. Disable "Chrome PDF Viewer" and enable the "Adobe Reader" option
